01. Information We Collect (PHI) 

We collect Personal Health Information (PHI) necessary to provide and coordinate high-quality home care services. This includes contact details, medical history, service records, and billing information. This data is collected directly from the client or their authorized family member during the assessment and care process. Centrixcare handles all PHI in compliance with the Health Insurance Portability and Accountability Act (HIPAA).

02. How We Use and Safeguard Your Information 

Your information is primarily used for Treatment, Payment, and Health Care Operations (TPO), which includes developing your care plan, processing payments, and internal quality assurance. We maintain administrative, technical, and physical safeguards to protect the confidentiality, integrity, and security of your electronic and paper PHI.

03. Sharing and Disclosure of Information

Centrixcare will only disclose your PHI without specific authorization when permitted by HIPAA for purposes of TPO (e.g., sharing records with your physician or pharmacy). We may also disclose information when legally required, such as in public health activities or in response to a court order. We never sell, rent, or trade your personal or health information.

03. Your HIPAA Rights and Choices

As a client, you have specific rights concerning your PHI under HIPAA. These include the right to access and copy your records, the right to request amendments to incorrect information, and the right to obtain an Accounting of Disclosures. You may also request restrictions on how we use or disclose certain information. To exercise these rights, please contact our designated HIPAA Privacy Officer.
